Uniform LogoPHYSICAL EDUCATION CLOTHING ORDER FORM (Only for Students in Grades 6 - 8)

Mandatory clothing for physical education includes a Forest Ridge Academy t-shirt, shorts, and an extra pair of gym shoes to be kept at school. Forest Ridge Academy sweatshirts are strongly recommended for PE classes when students go outdoors. 

PLEASE COMPLETE THIS FORM BY FRIDAY, JULY 25, to make sure students have their PE uniforms by the opening of school.
